Nursing Staff
Conducts patient observation, instructs patients on self-care, and perform skilled procedures such as IV therapy, in-home chemotherapy monitoring, ostomy care, catheter care, wound care, and laboratory collections such as urine, blood, and sputum samples.
Physical Therapists
Western Valley Home Health is contracted with several physical therapy departments in order to allow the client access to appropriate care. Physical therapy is essential in assisting the patient to regain mobility and function through planned exercise and treatment programs.
Speech/Language Pathologists
Pathologist works with patients to develop a special speech restoration programs and voice retraining exercises for patients who have lost their speech due to stroke or other disability.
Home Health Aides
Under the direct guidance of a registered nurse, assists patients with hygiene, meal preparation, errands, and other duties related to home convalescence.
Case Management / Elder’s Choice
Case manager will assist client with eligibility and the application process for Medicare and Medicaid services. Program helps with the home situation, financial resources, and availability of community resources. Elders Choice can provide homemaker, personal care aide service, and respite care to allow the client to remain in their home without the need for skilled nursing.
Payer Considerations
To qualify for home care under Medicare and Medicaid, the patient must be homebound, require a skilled need for nursing, physical and/or speech therapist and be under the care of an MD.
